**ALL PDE APPLICATION FORMS MUST BE SUBMITTED WITH ORIGINAL SIGNATURES.**

**BEFORE YOU SEAL THE ENVELOPE . . . Check off all of the required documentation
listed below and include with your application packet.**


1. A nonrefundable $80.00 U.S. Money Order fee (made payable to Commonwealth of Pennsylvania) for each certification area requested.

2. General Application, Form PDE 338 G. A separate PDE 338 G form and fee must be submitted for each certification area requested. FORM 338G

3. College/University Verification, Form PDE 338 A. PDE FORMS A & V

4. College transcripts (ALL undergraduate and graduate) from all colleges/universities attended in college-sealed,
unopened envelopes.

5. Verification of Experience, Form PDE 338 V. (One for each school district.) NOTE: Double check to make sure the
submitted form includes grade and subject area taught.

6. Photocopies of educator certificates or licenses (grades K-12) held in other states. If you are applying for an
instructional certificate under the basis of teaching three years within the past seven years, you must include the fully issued certificate(s) valid for the three years reported on the PDE 338 V form. Only service applicable to the area of certification you are applying for can be accepted.

7. Test scores must be reported to the Pennsylvania Department of Education officially either
directly from the test administrator - ETS or in ETS sealed unopened envelope or from ACTFL. It is recommended to include a photocopy of your ETS score report. It will be used to verify information with ETS if a problem should arise. NOTE: All tests should be completed prior to application.
