Art smock (usually an old oversized t-shirt works best. Please write your child's name in the collar.
Any personal care needs materials your child will need. (We can store extra shorts, socks, etc) Please label. Some parents have expressed that they prefer to keep a change of clothes in the bag. That way it is easily visible if they need a new change of clothes for school.
Looking at Afton's grade level supply list, there are not materials needed for inclusion subjects (except art) at the present time. I will let you know if there are specific needs as they come up.
Within A9 students will be provided with crayons, scissors, glue, etc. We will be going in for inclusion opportunities as appropriate with Mrs. Corbett's class. If you are able to send some of the following that would be great:
* 1 Small 8-12 pack of think washable markers
* 1 box of tissues
* 1 box of crayons 16-24 count
* 1 pair of pointed scissors
* 1 container of Clorax type wipes (any brand or store brand is fine)
* 1 colored pencil set (16 set)
* 1 pack of #2 pencils (12 pack if possible)

Emergency Information Card (one for the IU and the district tends to require their own form: sorry for the doubling).
Permission to Administer Meds form (if applicable) Instructions on sending meds to the nurse are on there.
Media Release Form: I use pictures and videos of the children throughout the day if you give permission. I also can make movies for the entire class, but I will go by what the media release says. I would like to do a password protected podcast page, but will ask for that permission separately.
Free and Reduced Lunch form (if applicable) will come home the first day of school. I will be sending the classroom info to the cafeteria manager to get the students' lunch PINs.
