For all Assistive Technology referrals:
Complete the District Referral Form and any related required forms (download from the District Referral Form list), then attach all forms to an email to the district special education supervisor. The supervisor will forward the request to the IU. This process must be followed for all Assistive Technology Referrals.
SETT Meeting Form This form is to be filled out by IU AT staff with teachers and other district staff at the AT meeting, or in preparation prior to the meeting. Please note this is not the AT Referral Form, which is required by the IU before assistive technology services may be provided.
